To establish care as a new patient call the office and select the option for becoming a new patient. A staff member will assist with collecting the needed information and scheduling your appointment. Patients that have access to a computer are encouraged to Pre-register as this reduces the time needed on the phone when calling. Patients will need to arrive early on the day of their appointment to fill out paperwork. We recommend arriving at least 20 minutes before your appointment. Patients also have the option of coming into the office and completing all of their paperwork before scheduling their appointment.
